TMS Training Reservation Request

Reservation Request Instructions

1. Please select the TMS Training class for which you are requesting reservations.

2. If you are requesting reservations for BOTH the Basic and Advanced classes you must submit two forms -- one for each class.

3. Fill out the form completely. Incomplete applications cannot be processed and will delay or prevent your reservation.

4. Reservations are taken on a "first come - first served" basis using the time of form submission to determine priority.

5. A waiting list will be established when all slots have been filled.

   Your submission of a reservation request does not guarantee a reservation in the class.
   Once we process your request we will send an email to the address you supply below.
   The email will either confirm your reservation or inform you that you have been placed on the waiting list.

   If you are unable to attend after making reservations, please send us an email to cancel the reservation
   so that we can release the space to someone on the waiting list.
   Please see our Cancellation Policy on the class flier for important cancellation deadlines.

* Prerequisite: You must have completed a Basic TMS Millennium class prior to attending the Advanced class.
** 10% Group discounts for 3 or more students attending from the same agency.
